GENERAL INFORMATION:

Health Screenings:

The district offers a variety of services to our students and their parents. Routine screenings are performed at recommended grade levels. Screenings are also available up on parent request if a concern is present. Parents are notified of findings that require a referral. Physical exams are required for new entrants, and for grades PreK or K, 1, 3, 5, 7, 9, and 11. Parents are asked to submit copies of recent physical exams by their private physician for those grade levels. If a copy is not received within 30 days of the start of school, an appointment will be made with the school physician to provide a physical during school hours. Athletic screenings also are performed by the school physician if there is no current copy of a private physical on file. In addition, the school physician is available to provide physicals for working papers in the HS health office.

2023 NYS School Heatlh Examination Form

Medication:

No medication, including non-prescription medications such as pain relievers, antacids or allergy medications, can be administered by a school nurse unless accompanied by a written prescription from a physician. The doctor must inform us in writing what the medicine is, the dosage and when the student is to take it. The medicine must be provided in the original container with prescription label attached. A permission slip to administer the medicine must also be provided by the parent/guardian, this form is available in the health office.

Parental permission also must be given for students to use cough drops and sunscreen in school and these items also must be kept in the Health Office.

Medical Records:

Remember, if you need immunization records for your child to enroll in summer camp, please call before June 15. Camp and daycare physicals should be done by your family physician.

Immunizations:

All enrolled students are required by law to be up to date on mandated immunizations (polio, diphtheria, measles, German measles (rubella) and mumps). The school district cannot admit new students who do not have doctor’s proof of these immunizations. Immunization requirements are monitored by the school nurses. All immunization records can be submitted to the school nurse your child attends. The students record needs to be verified and signed by your physician or provider. Failure to comply with these mandates may result in exclusion from school. If you need immunization records for college, camp or childcare, you may request a copy from the school's health office.

Children born on or after January 1, 1985, are required to show proof of two vaccines against measles.

Student Attendance

Regular attendance is essential if a student is to gain the full benefit of the school program. While attendance is required of all eligible students by law under certain specific conditions, students who attend regularly because they believe school is an important part of their lives and who see the value of preparing for life beyond school years seem to learn more and enjoy the experience.

Promote regular attendance. Make it a way of life for your children and help them gain as much as possible from the school program.
